This is a song sung by Lata Mangeshkar and Hariharan from the film Dil To Pagal Hai (1997) starring Shahrukh Khan, Madhuri Dixit, and Karisma Kapoor. The lyrics are beautiful and I have provided the Hindi and respective translations into English for the whole song (from here), as well as a youtube video of the song with english subtitles for those who wish to hear this sentimental 15-year-old song.
